What's up? = How are you? or How are you doing? You can also say, "What's up with you?" I use this everyday as an American, and now you can too!

This is a very casual expression used with friends, family and co-workers. The best answer to give:

"Nothing much, how about you?"

So here is the super simple mind blowing conversation:

"Hey man, what's up?"
"Not much, how about you?"
"Not much."
