 If someone says that for example, ALASKA is a country, I will assume that it is a mistake. If they do it over and over again, then I might assume that there is some agenda. 

CNN acknowledged that they made a n error mistake in calling it a country twice in dozens of stories about Tibet. That sounds like a mistake, and not malice.
