To "hold something over someone's head" means to use something, like a piece of information or something from someone's past, against them.

For example, imagine that a man cheats on his wife by kissing another woman. His wife finds out and is very angry at him. But he apologizes and does everything he can to earn his wife's forgiveness. Eventually, she forgives him. But even though he has earned her forgiveness, she continues to remind him about the time he cheated on her. She uses this to get what she wants from him, win an argument, or make him feel guilty. She "holds it over his head."
