1) “It is I who is responsible for the mismanagement” is correct English, though it sounds very formal. It would be more natural simply to say “I am responsible for the mismanagement.”


As for your fill in the blank question, the correct answer is “We have yet to decide what action to take.”

I have got something to clarify here. According to the grammar book I have it says :  3) It is I who am responsible for the mismanagement.- is the correct option. It also says that since ‘I’ is the antecedent of ‘who’ and it defines the relative pronoun, therefore the helping verb should be in accordance with the pronoun ‘I”. For example, in place of the pronoun ‘I’ had it been ‘he’/’‘she’ or ‘they the helping verb would have been ‘is’ and ‘are’ respectively. What is your take on that?


e.g. A) It is he/she who is responsible for the mismanagement


      B) It is they who are responsible for the mismanagement

Was #2 supposed to read “It is I who am responsible for the mismanagement?” Your sentence is missing the word “I.” I was confused by that choice, but now I understand!


Your grammar book is correct. But again, saying “I am responsible for the mismanagement” sounds more natural.
